If your parent is being discharged
You likely have 48 hours. Here is what to do.
Discharge planners often give families very little notice, and the list they hand you is long. We can usually complete a nurse assessment within twenty-four hours and accept a move-in within forty-eight.
Call rather than email if you are working to a date. One of the owners answers this phone.

What happens
Four steps, usually inside two days.
You call, and we talk properly
What happened, what she needs now, and what the discharge planner has said. If we’re not the right fit, we’ll say so and point you somewhere better. No cost, and no sales calls.
Our RN assesses her where she is
At the hospital, at rehab, or at home. You don’t need to bring her here first. If she is on Medicaid, bring her CARE assessment if you have it.
We give you a straight answer and a real number
What care she needs, what it costs, and what is included. Usually the same day.
She moves in, and the paperwork follows
Her written care plan is completed together with you in the weeks after. It doesn’t have to be finished before she has somewhere safe to sleep.
